  1. When we charge a student a fee for a lost or damaged Chromebook, hotspot, calculator, charger or hotspot case, complete this form.
  2. Send the student to Cashiering with the form. 
  3. Once they return with the paid receipt, you need to mark the item as paid for in Sierra.
  4. We do not keep the receipt from Cashiering, please return that to the student.
